Target Audience: Pharmacists in any practice setting, particularly the community setting.

Why Attend: A review of current literature emphasizes the integration of social determinants of health (SDOH) into routine pharmacy practice to improve patient outcomes. Addressing SDOH can reduce health disparities and enhance medication adherence, yet implementation remains inconsistent across community pharmacy settings. Community pharmacists report barriers to SDOH integration including lack of standardized workflows, limited training, and uncertainty about reimbursement. This activity aims to equip pharmacists and members of the pharmacy team with actionable tools to close these gaps and align with emerging best practices. 

Objectives: 

  1. Describe the impact of Social Determinants of Health (SDOH) on medication adherence and patient outcomes.
  2. Recognize validated tools and workflow strategies for integrating SDOH screening into patient encounters.
  3. Outline effective referral pathways to community resources that address identified SDOH needs.

Program Schedule: 

12:00-12:05 pm: Introduction

12:05-12:50 pm: Educational presentation

12:50-1:00 pm: Q&A and learning assessment

Requirements for Obtaining Continuing Education Credit:

Learners must be present for 70% or more of the webinar. Please ensure your screen name accurately identifies you. The activity evaluation, which includes learning assessment questions, is a requirement for CPE credit to be awarded. There is no minimum passing score required for the learning assessment questions. It is strongly encouraged that all learners participate in the active learning components of the program to gain the full benefit of the educational activity.

For live webinar activities, no requests for refunds will be honored after educational materials have been disseminated to registered attendees (typically 24 hours prior to the program start). Accreditation Information:
